How to complete your adult autism pre-assessment forms

Before you can be added to our waitlist for an autism assessment, we ask that you complete some important pre-assessment forms. These forms are essential in providing our clinicians with the information they need to understand your unique situation and help us reach an accurate diagnostic outcome.

We understand that this process might feel a bit daunting, so we've created a step-by-step guide to help you navigate it. Below, you'll find everything you need to know to access, complete, and submit your forms through your ProblemShared client dashboard.  

Accessing your forms  

  1. Sign in. Start by signing into your ProblemShared client dashboard.
  1. Get started. Click the “Start” button to go to the pre-assessment forms page.
  1. Upload I.D. Next, you need to upload two documents: proof of address and photographic identification. Some examples of acceptable documents are featured in drop-down menus.
  1. Provide informant details. An informant is a person nominated to provide further information to our clinicians that will contribute to your assessment. To learn more about who you can choose to be your informant, you can read this support article. Please provide the details of your informant in the ‘Informant details’ section.
  1. Download forms. Click the “Download” button next to each form, which will open the form in a new tab. On the menu bar at the top of your screen, select File > Save As to download a PDF copy of each form onto your computer. There are two forms to complete for an adult autism assessment, these are the Pre-Assessment Questionnaire and the Informant Questionnaire.
  1. Complete forms. Use Adobe Acrobat to open each form, just like you did with your self-report form at the referral stage of your journey with us. At the top of each form, you can see who needs to complete it.  
  1. Upload forms. Once you’re finished filling out each form, you need to upload them back onto your client dashboard so we can access them. Click the “Upload” button next to the correct form, and select the saved file.

Completing your forms

1. Pre-Assessment Questionnaire

  • Who should complete this form?  This form should be completed by you, the person seeking an assessment.
  • What is the purpose of this form? This form is a chance for you to report on your developmental history and current experience in various settings.
  • How do I complete this form? Start by providing personal details at the top and work through each of the sections, answering the questions as thoroughly as possible.
  • How do I upload this form?  Once you’re finished, upload the form back onto your client dashboard.

2. Informant Questionnaire

  • Who should complete this form? This form needs to be completed by your nominated informant. You must download the form and send it to your informant for them to complete.
  • What is the purpose of this form? This form is a chance for your informant to provide our clinicians with some helpful context about your early development and current symptoms of autism.  
  • How do I upload the form?  Once your informant has finished filling out the form, they must send it back to you. Then, you can upload it back onto your client dashboard.

Additional tips  

  • Make sure you’ve got the right tech. Ensure that you have access to a suitable device for completing these forms, such as a laptop or tablet. If you do not have access to a suitable device, it may be possible to fill them out at your child’s school and borrow a school computer. However, this would be reliant on you asking the school and checking that you can log onto your ProblemShared client dashboard successfully, in case there are security issues with firewalls.
  • Take your time. There is a lot of information in the forms, and we appreciate that this could seem overwhelming. Consider breaking each form down into smaller sections, and use bullet points instead of long paragraphs if that’s easier.  
  • Don’t worry about health information. You do not need to have your child’s blood pressure, weight, or height details before the assessment. These can be collected later if necessary.
  • Password reset guidance. If you reset the password to your client dashboard but can’t find the email in your inbox, remember to check your spam folder.
  • Double-check your forms. You can view each uploaded form on your client dashboard to make sure each one is correct.

Final note
